About Us:

Link-Up Detroit is an outreach initiative within the Detroit Health Department’s HIV/STI Division. Link-Up Detroit aims to provide linkage services for persons living with HIV (PLWH) seeking HIV care, EIS/DIS services, and/or housing resources with agencies within the Detroit Eligible Metropolitan Area (DEMA). 

Our Mission:

The mission of the Detroit Health Department is to address public and population health priorities of Detroiters.

  • Our program seeks to help every HIV positive Detroiter get the medical and social support they need to achieve viral suppression and live a happy and healthy life.

What we do:

Link-Up Detroit can help HIV-positive individuals access medical and non-medical services, including:

  • Transportation
  • Medical Case Management
  • Early Intervention Services (EIS)
  • Support Groups
  • Health Insurance Assistance
  • Co-Pay Assistance
  • Insurance Enrollment
  • Job Training
  • Food Assistance
